ABSTRACT

Introducción. El cáncer de cuello uterino es un problema de salud pública relevante en países de ingresos medios y bajos. El seguimiento de mujeres con tamización positiva y el acceso a tratamiento para neoplasia intraepitelial cervical (NIC) son retos mayores en estos países. Objetivo. Evaluar la efectividad de la crioterapia suministrada por enfermeras en casos de neoplasia intraepitelial de cérvix. Materiales y métodos. Se hizo la inspección visual directa con ácido acético y solución yodada (VIA-VILI), y se practicó colposcopia con biopsia, a mujeres entre los 25 y los 59 años, residentes en zonas de bajos ingresos de Bogotá. Profesionales de enfermería entrenados ofrecieron tratamiento inmediato con crioterapia a mujeres positivas en la inspección visual. Se les practicó colposcopia con biopsia antes del tratamiento y en un control a los 12 meses. Se evaluó la efectividad mediante tasas de curación (resultado: sin lesión) y regresión de NIC2/3 (resultado: ≤NIC1), por verificación colposcópica e histológica. Resultados. Se tamizaron 4.957 mujeres. En total, 499 fueron positivas y 472 aceptaron el tratamiento inmediato. Recibieron crioterapia por enfermería 365 mujeres (11 NIC2/3). La tasa de curación fue del 72 % (IC95%: 39-94 %) por verificación colposcópica, y del 40 % (IC95%: 22-85 %) por histología. Las tasas de regresión fueron del 100 y el 60 %, respectivamente. Se reportaron dos eventos adversos no graves relacionados. Conclusiones. Las tasas de curación y regresión por verificación colposcópica son similares a las reportadas con crioterapia administrada por médicos. El tamaño de la muestra con NIC2/3 dificulta la comparación por tipo de verificación. Los hallazgos apoyan la implementación de estrategias de "ver y tratar" por parte de enfermería en poblaciones con acceso limitado a servicios de salud.


Introduction. Cervical cancer is a relevant public health problem for low- and middleincome countries. Follow-up of positive-screened women and compliance with treatment of precancerous lesions are major challenges for these settings. Objective. To evaluate the efficacy of cryotherapy delivered by nurses for cervical intraepithelial neoplasia (CIN). Materials and methods. Direct visual inspection with acetic acid and lugol iodine (VIA- VILI), and colposcopy/biopsy were performed on women 25 to 59 years old, residents of low-income areas in Bogotá, Colombia. Trained nurses offered immediate cryotherapy to every woman with positive visual inspection. Colposcopy/biopsy was performed before treatment and at a 12-month follow-up. The effectiveness was measured as cure (outcome: no-lesion) and regression (outcome: CIN1) rates of CIN2/3 using colposcopic and histological verification. Results. A group of 4.957 women with VIA/VILI was valuated. In total, 499 were screen positive and 472 accepted immediate treatment. A total of 365 women (11 CIN2/3) received cryotherapy by nurses. Cure rate was 72% (95%CI: 39%-94%) and 40% (95%CI: 22%- 85%) by colposcopic and histological verification, respectively. Regression rates were 100% and 60%. There were two related non-serious adverse events. Conclusions. Cure and regression rates by colposcopic verification are like those reported for cryotherapy delivered by doctors. The sample size (CIN2/3) hinders comparisons by type of verification. Our findings support the implementation of screen-and-treat algorithms by nurses among populations with limited access to health services.

ABSTRACT

ABSTRACT We report two cases of stage 3A unilateral Coats' disease in pediatric patients. In both cases, disease control was achieved using a dexamethasone intravitreal implant in addition to other treatments. The treatment improved visual acuity in one patient and prevented the worsening of the decline in visual acuity in the other patient during follow-up periods of 7 and 3 years, respectively. One of the patients presented an increase in intraocular pressure, which was controlled with topical antiglaucoma medication, but developed a cataract that required surgery. In conclusion, dexamethasone intravitreal implant may be a useful adjuvant treatment to consider in some pediatric cases with Coats' disease.


RESUMO Relatamos dois casos de doença de Coats em estágio 3A unilateral em pacientes pediátricos. Em ambos os casos, o controle da doença foi obtido com implante intravítreo de dexametasona, além de outros tratamentos, com melhora da acuidade visual em um caso e sem piora da visão no outro, durante um período de acompanhamento de 7 e 3 anos. Um dos casos apresentou elevação da pressão intraocular controlada com medicação antiglaucoma tópica e desenvolveu catarata que exigiu cirurgia. Em conclusão, o implante intravítreo de dexametasona pode ser um tratamento adjuvante útil a ser considerado em alguns casos pediátricos com doença de Coats.

The stretching with thermotherapies associated have been related in the literature as a way to increase the range of motion (ROM), but still are not consensus about this efficacy and costeffectiveness. The muscle stretching is a useful technique in rehabilitation and in physical activities programs, either to gain or maintain the flexibility, as a recovery of musculoskeletal and joint injuries. Objective: To determine the effects of thermotherapies associated to flexibility training in ROM of knee extension in healthy adults. Method: The search strategy was conducted in main databases, as Cochrane Library, LILACS, PEDro, PUBMED/ Medline, Scopus and Web of Science. The searches were done in 2016, and renewed in 2023, in order to achieve new publications along this time. Were selected only randomized clinical trials that have executed a training of hamstrings stretching, associated or not with one or more thermotherapies in healthy young adults, as long as the outcome was knee extension ROM. Only papers in Portuguese or English were assessed. To evaluate risk of bias was used the Cochrane Collaboration's Risk of Bias Tool, and the methodological quality assessment was rated following the PEDro Scale. Results: Eight articles were included, totalizing 260 participants. The papers showed low methodological quality, and uncertain risk of bias. Only chronic effect of local warming and cryotherapy plus stretching showed a statistically significant difference versus control group. However, the thermotherapy action associated with stretching is still unclear, once the results suggest that even without the thermotherapy there are ROM increases. Conclusion: The stretching is effective in knee extension ROM improvement in healthy adults, with or without thermotherapy. New studies with higher methodological rigor and standardized protocols are needed


Os alongamentos com termoterapias associadas têm sido relatados na literatura como forma de aumentar a amplitude de movimento (ADM), mas ainda não há consenso sobre sua eficácia e custo-efetividade. O alongamento muscular é uma técnica útil na reabilitação e em programas de atividades físicas, tanto para ganho ou manutenção da flexibilidade, quanto para recuperação de lesões musculoesqueléticas e articulares. Objetivo: Determinar os efeitos das termoterapias associadas ao treinamento de flexibilidade na ADM de extensão do joelho em adultos saudáveis. Método: A estratégia de busca foi realizada nas principais bases de dados, como Cochrane Library, LILACS, PEDro, PUBMED/ MedLine, Scopus e Web of Science. As buscas foram realizadas em 2016, e renovadas em 2023, a fim de alcançar novas publicações ao longo deste tempo. Foram selecionados apenas ensaios clínicos randomizados que tenham executado um treinamento de alongamento de isquiotibiais, associado ou não a uma ou mais termoterapias, em adultos jovens saudáveis, desde que o desfecho fosse ADM de extensão de joelho. Apenas artigos em português ou inglês foram avaliados. Para avaliar o risco de viés foi usado o Risk of Bias Tool da Cochrane Collaboration, e a avaliação da qualidade metodológica foi classificada de acordo com a Escala PEDro. Resultados: Foram incluídos oito artigos, totalizando 260 participantes. Os artigos apresentaram baixa qualidade metodológica e risco incerto de viés. Apenas o efeito crônico de aquecimento local e crioterapia associados ao alongamento mostrou uma diferença estatisticamente significativa em relação ao grupo controle. No entanto, a ação da termoterapia associada ao alongamento ainda não está clara, uma vez que os resultados sugerem que mesmo sem a termoterapia há aumento da ADM. Conclusão: O alongamento é eficaz na melhora da ADM de extensão do joelho em adultos saudáveis, com ou sem termoterapia. Novos estudos com maior rigor metodológico e protocolos padronizados são necessários

SUMMARY OBJECTIVE: This study aimed to evaluate the effectiveness of cryotherapy in elderly patients with rib fractures due to blunt thoracic trauma. METHODS: In this prospective randomized controlled study, geriatric patients were assigned to groups to receive either coolant spray (n=51) or placebo spray (n=50). The visual analog scale scores of all patients were recorded before starting spray application (V0), as well as at 10th (V1), 20th (V2), 30th (V3), 60th (V4), 120th (V5), and 360th (V6) minute. The mean decreases in the visual analog scale scores were calculated. RESULTS: The differences between V0 and V1, V0 and V2, V0 and V3, and V0 and V4 mean visual analog scale scores measured in the coolant spray group were found to be significantly higher (p<0.001). In V1, V2, V3, and V4 measurements, the incidence of "clinical effectiveness" in the coolant spray group was significantly higher than in the placebo group (p=0.001). CONCLUSIONS: Coolant spray therapy can be used as a component of multimodal therapy to provide adequate analgesia due to rib fractures in geriatric patients.

ABSTRACT

Introducción: En la actualidad se emplean nuevas alternativas no invasivas para la remoción de diversos procesos inflamatorios del epitelio mucoso bucal, siendo la crioterapia una técnica fisioterapéutica basada en la aplicación del frío intenso con nitrógeno líquido para producir en la piel o mucosas del organismo una congelación y destrucción local de células y tejidos afectados de forma controlada. Objetivo: Evaluar los efectos terapéuticos de la criocirugía en pacientes geriátricos portadores de estomatitis subprótesis. Métodos: Se efectuó un estudio descriptivo y transversal de 50 pacientes adultos mayores, de ambos sexos, diagnosticados con estomatitis subprótesis y asistidos en la consulta estomatológica del Policlínico de Especialidades referente al Hospital Clinicoquirúrgico Universitario Saturnino Lora Torres de la provincia Santiago de Cuba, en el periodo comprendido desde marzo de 2022 hasta igual mes de 2023. Resultados: Se evidenció que las congelaciones entre 20 y 30 segundos y los deshielos entre 91 y 120 segundos fueron los más eficaces en la destrucción de la mucosa palatina dañada, logrando la regeneración tisular en un tiempo mínimo con la indicación de uno a tres ciclos de enfriamientos rápidos y deshielos lentos en una sesión terapéutica. La ausencia de síntomas fue lo más relevante en la evolución postquirúrgica. Conclusiones: La crioterapia demostró ser un método eficaz e inocuo para regenerar los tejidos lesionados al realizar congelaciones rápidas con deshielos lentos. Este método crioquirúrgico, fácil de aplicar y atraumático, logró una óptima reparación y reepitelización de los tejidos bucales dañados con excelentes respuestas terapéuticas, evitando complicaciones postquirúrgicas como la infección y el sangramiento local.


Introduction: At present, new non-invasive alternatives are used for the removal of various inflammatory processes of the oral mucous epithelium; being cryotherapy a physiotherapeutic technique based on the application of intense cold with liquid nitrogen to produce freezing on the skin or mucous membranes of the organism and local destruction of affected cells and tissues in controlled manner. Objective: To evaluate the therapeutic effects of cryotherapy in geriatric patients with sub-prosthesis stomatitis. Methods: A descriptive and cross-sectional study of 50 older adult patients, of both sexes, clinically diagnosed with subprosthesis stomatitis and assisted in the dentistry consultation of the Specialties Polyclinic referring to the Saturnino Lora Torres University Clinical Surgical Hospital in the city of Santiago de Cuba, in the period from March 2022 to the same month of 2023. Results: It was evidenced that freezing between 20 and 30 seconds and thawing between 91 and 120 seconds were the most effective in the destruction of the damaged palatal mucosa, achieving tissue regeneration in a minimum time with the performance of 1 to 3 cycles of rapid cooling and slow thawing in a therapeutic session. The absence of symptoms was the most relevant in the post-surgical evolution. Conclusions: Cryotherapy proved to be an effective and innocuous method to regenerate injured tissues when fast freezing with slow thawing. This cryosurgical method, easy to apply and atraumatic, achieved optimal repair and re-epithelization of damaged oral tissues with excellent therapeutic responses, avoiding post-surgical complications such as infection and local bleeding.

ABSTRACT

Prostate cancer is now the second most common malignancy in men worldwide, with an increasing incidence in China. Most prostate cancer patients receive whole-gland therapy after diagnosis, but patients with localized prostate cancer may not benefit from the treatment due to side effects. With the development of imaging technology and the theory of "index lesion," focal therapy has been greatly developed, which includs high intensity focused ultrasound, focal laser ablation, cryotherapy, irreversible electroporation and photodynamic therapy. This study reviews the clinical trials in recent years and reveals that high intensity focused ultrasound and focal laser ablation have better failure-free survival and postoperative functional control compared with other focal therapy techniques.

El virus del papiloma humano es una infección prevalente, que puede infectar cualquier mucosa del cuerpo y causar verrugas genitales externas o condilomas genitales y cáncer de cuello uterino. El tratamiento es difícil con una alta recurrencia y persistencia de las mismas, lo que afecta mayormente a mujeres jóvenes. El objetivo del presente estudio fue realizar una comparación entre inmunomoduladores y crioterapia para el tratamiento de lesiones genitales en mujeres con VPH. Se realizó una revisión bibliográfica de la literatura científica, entre inmunomoduladores y crioterapia para el tratamiento de lesiones genitales en mujeres con VPH de los últimos 20 años, donde se identificaron publicaciones de revisiones sistemáticas y metaanálisis. Se concluyó que la elección de los inmunomoduladores al igual que la crioterapia es mejor utilizarlos cuando existe lesiones clínicas inducidas por el VPH en la región genital y perianal en mujeres, dependiendo de la cantidad, el tamaño, la gravedad, la ubicación de las verrugas y las preferencias del paciente.


Human papillomavirus is a prevalent infection, which can infect any mucosa of the body and cause external genital warts or genital warts and cervical cancer. Treatment is difficult with a high recurrence and persistence of the same, which mainly affects young women. The objective of the present study was to compare immunomodulators and cryotherapy for the treatment of genital lesions in women with HPV. A narrative bibliographic review of the scientific literature was carried out, between immunomodulators and cryotherapy for the treatment of genital lesions in women with HPV of the last 20 years, where publications of systematic reviews and meta-analyses were identified. It was concluded that the choice of immunomodulators, like cryotherapy, is better used when there are clinical lesions induced by HPV in the genital and perianal region in women, depending on the number, size, severity, location of the warts and patient preferences.


O papilomavírus humano é uma infecção prevalente, que pode infectar qualquer mucosa do corpo e causar verrugas genitais externas ou verrugas genitais e câncer cervical. O tratamento é difícil com alta recorrência e persistência da mesma, que acomete principalmente mulheres jovens. O objetivo do presente estudo foi comparar imunomoduladores e crioterapia para o tratamento de lesões genitais em mulheres com HPV. Foi realizada uma revisão bibliográfica narrativa da literatura científica, entre imunomoduladores e crioterapia para tratamento de lesões genitais em mulheres com HPV dos últimos 20 anos, onde foram identificadas publicações de revisões sistemáticas e metanálises. Concluiu-se que a escolha de imunomoduladores, como a crioterapia, é melhor utilizada quando há lesões clínicas induzidas pelo HPV na região genital e perianal em mulheres, dependendo do número, tamanho, gravidade, localização das verrugas e preferências da paciente.

ABSTRACT

Introdução: Habitualmente, as verrugas virais são causadas pelo papilomavírus humano (HPV) e, embora tenham tendência à resolu- ção espontânea, quando crônicas, conferem resistência terapêutica. Objetivo: Divulgar um caso de sucesso terapêutico da associação do uso de imiquimode e criocirurgia para verrugas virais periungueais e subungueais. Relato do caso: O relato é de um homem de 56 anos, com cinco anos de evolução de verrugas subungueais e periungueais distribuídas na maioria dos dedos das mãos. Anteriormente, foi tratado com antifúngicos pelo resultado micológico, sem sucesso. Foi submetido à biópsia subungueal, cujo anatomopatológico confirmou verruga plana. Foi proposto o tratamento combinado de imiquimode creme 5% e criocirurgia. Evoluiu com resolução completa das lesões virais. Conclusão: Os autores demonstram a importância dos achados anatomopatológicos no diagnóstico de verrugas virais recalcitrantes, bem como é ressaltada a eficácia da associação do uso de imiquimode e criocirurgia.


Introduction: Viral warts are usually caused by the human papillomavirus (HPV), and although they tend to resolve spontaneously, when chronic, they confer therapeutic resistance. Aim: To report a case of therapeutic success of imiquimod and cryosurgery combination for periungual and subungual viral warts. Case Report: The report is of a 56-year-old man with five years of evolution of subungual and periungual warts distributed on most fingers of the hands. Previously, he received treatment with antifungal drugs due to the mycological result, but it was unsuccessful. He underwent a subungual biopsy, whose pathology confirmed the flat wart. A combined treatment of imiquimod cream 5% and cryosurgery was proposed. The patient evolved with complete resolution of the viral lesions. Conclusion: The authors demonstrate the importance of pathological findings in the diagnosis of recalcitrant viral warts, as well as the efficacy of the association of imiquimod and cryosurgery.

ABSTRACT

Introducción: El trauma grave constituye una de las principales causas de muerte y discapacidad. Si bien es una enfermedad muy heterogénea en cuanto a su origen, tipos lesionales y gravedad, existe gran incertidumbre en cuanto a su pronóstico. Objetivo: Evaluar la efectividad de la crioterapia y la electroestimulación nerviosa transcutánea para la disminución del dolor y el edema en deportistas con afecciones del sistema osteomioarticular. Métodos: Se realizó un estudio observacional, de corte descriptivo y retrospectivo, de 5 548 deportistas con afecciones del sistema osteomioarticular, que acudieron al Servicio de Rehabilitación Física del Estadio José Antonio Huelga de la provincia de Sancti Spíritus, entre noviembre de 2018 y diciembre de 2019, para recibir crioterapia y electroestimulación nerviosa transcutánea para la disminución del edema y el dolor. Entre las variables estudiadas figuraron: sexo, edad, tipo de deporte que practicaban, lesiones que presentaban, recuperación y tiempo de desaparición de los síntomas. Resultados: Predominaron el sexo masculino (66,83 %), el grupo etario de 8-21 años (57,33 %), el futbol y el atletismo como deportes con mayor número de afectados (23,08 y 22,85 %, respectivamente), así como los esquinces (56,54 %) y las fracturas (20,81 %) como lesiones principales. Conclusiones: La terapia aplicada resultó efectiva y segura para disminuir el edema y el dolor en los deportistas a causa de enfermedades traumáticas del sistema osteomioarticular.


Introduction: The serious trauma constitutes one of the main causes of death and disability. Although it is a very heterogeneous disease as for its origin, types of injure and seriousness, great uncertainty exists as for its prognosis. Objective: To evaluate the effectiveness of cryotherapy and the nervous transcutaneous electroestimulation for the decrease of pain and edema in sportsmen with affections of the osteomyoarticular system. Methods: An observational, descriptive and retrospective study of 5548 sportsmen with affections of the osteomyoarticular system was carried out, they went to the Physical Rehabilitation Service of the José Antonio Huelga Stadium in Sancti Spíritus, between November, 2018 and December, 2019, to receive cryotherapy and nervous transcutaneous electroestimulation for the decrease of edema and pain. Among the studied variables figured: sex, age, type of sport that they practiced, injure that presented recovery and time of disappearance of symptoms. Results: There was a prevalence of the male sex (66.83 %), the 18-21 age group (57.33 %), football and athletics as sports with a higher number of affected patients (23.08 and 22.85 %, respectively), as well as sprains (56.54 %) and fractures (20.81%) as main lesions. Conclusions: The applied therapy was effective and safe to diminish the edema and pain in sportsmen due to traumatic diseaes of the osteomyoarticular system.

ABSTRACT

Aim: The aim of this study was to evaluate the intracanal effectiveness of cryotherapy, curcumin irrigant, and normal saline as a final irrigant in reducing postendodontic pain in primary teeth. Materials and Methods: A total of 120 teeth between the ages of 4 and 7 years requiring pulpectomy in primary teeth were included in the study. The teeth were randomly assigned to one of the three treatment groups: intracanal cryotherapy using 2.5°C cold saline, curcumin irrigant, or normal saline. Following completion of chemomechanical preparation, final irrigation with 2.5°C cold saline, curcumin irrigant, and normal saline solution at room temperature were employed in the groups. Participants were asked to rate the severity of their postoperative pain on the Visual Analog Scale before, immediate postoperative after wearing of local anesthetic effect, and 24 h after the procedure. The results were analyzed statistically. Results: The differences in reduction of postendodontic pain between the three irrigating regimens were statistically not significant. Cryotherapy utilizing 2.5°C cold saline or curcumin irrigant can be used instead of normal saline as a final irrigant in pulpectomy of primary teeth. Conclusions: Cryotherapy can be a straightforward, cost-effective, and nontoxic treatment option for the management of postendodontic pain. Curcumin irrigant with its anti-inflammatory properties is also a better alternative as a final irrigant for reducing postoperative pain in primary teeth.

Objetivo: Investigar os efeitos da associação entre alongamento e recursos termoterapêuticos sobre o ganho de flexibilidade e amplitude de movimento. Métodos: Este ensaio clínico, controlado, não randomizado e cego teve duração de três semanas, com duas sessões semanais e incluiu 27 indivíduos do sexo feminino, as quais compuseram tanto o grupo experimental quanto o controle, de forma que um de seus membros inferiores recebeu a intervenção terapêutica dos recursos termoterapêuticos e o membro oposto foi submetido apenas ao alongamento. A análise angular foi realizada através do software de avaliação postural Kinovera. Resultados: Os efeitos do alongamento associado ao calor prévio e resfriamento pós intervenção se mostraram mais eficazes comparado ao grupo controle pré-intervenção (p<0,000), grupo controle pós intervenção (p= 0,003) e grupo experimental pré-intervenção (p<0,000). Conclusão: A aplicação de calor prévio ao alongamento, seguido pelo emprego imediato de resfriamento propicia ganhos de flexibilidade muscular e amplitude de movimento maiores que o alongamento simples.


Objective: To investigate the effects between stretching and thermotherapeutics resources on gaining flexibility and range of motion. Methods: This controlled, non- randomized, blinded clinical trial lasted three weeks, with two weekly sessions and included 27 female individuals, who made up both the experimental and the control group, so one of their legs received therapeutic intervention from chemotherapeutic resources and the opposite one was submitted to stretching only. Angular analysis was performed using the Kinovera postural assessment software. Results: The effects of stretching associated with pre-intervention heat and post-intervention cooling were more effective compared to the pre-intervention control group (p<0.000), post- intervention control group (p= 0.003) and pre-intervention experimental group (p<0.000). Conclusion: The application of heat prior to stretching followed by immediate cooling provides gains in muscle flexibility and range of motion greater than simple stretching.

ABSTRACT

Historically cryotherapy, the application of cold for treatment has been used to manage pain and in?ammation. Its role in medicine and dentistry is well established while its applications in endodontics is promising. ABSTRACT Purpose: To examine changes in the eyes after cold compress application at the early stage. Method: A total of 62 eyes from 62 healthy adult participants were included in this cross-sectional and comparative study. The subfoveal choroidal thickness and retinal nerve fiber thickness were measured by spectral-domain ocular coherence tomography. The central corneal thickness, anterior segment volume and depth, iridocorneal angle, and pupil diameter were measured by means of the Scheimpflug anterior segment imaging method. The measurements were repeated after 10 min of cold compress application, which was applied using special packs. The procedures were then repeated with non-cold packages to exclude the effect of pressure. Results: The average age of the participants was 30.74 ± 5.82 years. There was no significant change in the central corneal thickness after cold compress application, and there was a significant decrease in the anterior segment volume (p<0.001), anterior segment depth (p<0.001), and pupil diameter. Moreover, the iridocorneal angle increased (p=0.002). The subfoveal choroidal thickness decreased after the application of cold compress (p<0.001). The overall disk thickness (p=0.034) and superior nasal scale (p=0.007) significantly decreased after the cold compress was administered during the evaluation of optic nerve fiber thickness. In contrast to that with the cold application, the subfoveal choroidal thickness and optic nerve fiber thickness did not change after the non-cold compress application (p>0.05). Conclusion: Cold compress application may thus cause some physiological changes in the eyes, which necessitates the examination of its usage and effects.


RESUMO Objetivos: Examinar as mudanças nos olhos após a aplicação com compressa fria. Método: Sessenta e dois olhos de 62 adultos saudáveis foram incluídos neste estudo transversal e comparativo. A espessura da coróide subfoveal e a espessura da fibra nervosa retiniana foram mensuradas por tomografia de coerência óptica de domínio espectral (OCT). A espessura central da córnea, o volume e a profundidade do segmento anterior, o ângulo iridocorneano e o diâmetro da pupila foram mensurados por meio do método de imagem do segmento anterior de Scheimpflug. As medições foram repetidas após 10 minutos de aplicação de compressas frias, aplicadas com embalagens especiais. Os procedimentos foram repetidos com embalagens não frias para excluir o efeito da pressão. Resultados: A média de idade dos participantes foi de 30,74 ± 5,82 anos. Embora não tenha havido alteração significativa na espessura central da córnea após a aplicação da compressa fria, houve diminuição significativa no volume do segmento anterior (p<0,001), na profundidade do segmento anterior (p<0,001) e no diâmetro da pupila. Além disso, o ângulo iridocorneano aumentou (p=0,002). A espessura da coróide subfoveal diminuiu após a aplicação da compressa fria (p<0,001). A espessura total do disco (p=0,034) e a escala nasal superior (p=0,007) diminuíram significativamente após a administração da compressa fria durante a avaliação da espessura da fibra do nervo óptico. Ao contrário da aplicação com compressa fria, a espessura da coróide subfoveal e a espessura da fibra do nervo óptico não mudaram após a aplicação da compressa não fria (p>0,05). Conclusão: A aplicação de compressa fria pode causar algumas alterações fisiológicas nos olhos e o seu uso e efeitos devem ser observados.

Purpose: To evaluate the effect of direct cell injury of cryotherapy on eyelid sebaceous gland carcinoma cells by an ex vivo cryotherapy experiment. Methods: It was a prospective interventional case series. Six patients with biopsy?proven nodular sebaceous gland carcinoma were included. After excision of the mass, a thin slice of the mass resembling the thickness of the conjunctiva was shaved off and was oriented over the broad end of a tissue forceps. Cryotherapy was applied to both its anterior and posterior aspects by the triple freeze?thaw technique. The mass was then labeled and sent separately for histopathological evaluation by fixation and staining. Results: A total of six patients with a mean age of 58.2 ± 15.5 years were included. There were four females and two males. The mean duration of the lesion was 21.6 ± 17.51 months. All patients had involvement of the upper eyelid. The patients were clinically staged as T2b (n=2), T1a (n=2), T2c (n=1), and T3a (n=1) respectively. There was no regional lymphadenopathy or metastasis in any of the cases. The experimental cryo?tissue containing the cryo?treated lesion revealed the presence of viable tumor cells (>50%) in all six specimens. Conclusion: The direct cell injury caused by cryotherapy may not be sufficient to kill all the residual sebaceous gland carcinoma cells on the tumor bed.

ABSTRACT

BACKGROUND Cervical cancer is the fourth most fatal and common disease globally among women of reproductive age in Kenya; it ranks the second most frequent type of cancer after breast cancer. Due to the high burden, cryotherapy treatment services, which are effective for the treatment of precancerous lesions are available in selected health facilities in Kenya, however, barriers to the treatment services are poorly understood. Nonetheless, understanding these barriers is critical for enhanced service delivery. MATERIALS AND METHODS :A descriptive facility-based cross-sectional study design was carried out to determine the barriers to cryotherapy treatment services among 60 women of reproductive age on a one-year therapy at Migosi Sub County Hospital in Western Kenya. The participants were selected purposively and interviewed via telephone calls using pre-coded semi-structured questionnaires. However, data from 5 nurses working in the cryotherapy section were collected through face-to-face interviews at the health facility. Data were entered in an excel sheet and then exported to SPSS version 23.0 for analysis. Both descriptive and inferential statistics (Chi-square) were used and data were presented in form of tables. RESULTS :Overall, 52 (85.4%) respondents adhered to post-care treatment instructions and reported no adverse reactions. However, 28 (46.7%) experienced unavailability of cryotherapy services at the time of the appointment and got the services later, 24 (40%) got the services at the time of the appointment but waited for a long time before being served, 37 (61.7%) did not know why they were being treated and 46 (76.7%) had misconceptions and myths about the therapy. In addition, there was a statistically significant association between knowing both the benefits of screening and cryotherapy [X 2 (1, N = 60) = 5.90, p = .02]. Also, the knowledge of the benefits of cryotherapy did not influence one's decision to wait for cryotherapy treatment services, [X 2(1, N = 60) = 3.98, p = .46]. CONCLUSION : The study shows very good adherence to post-treatment instruction but inadequate availability of cryotherapy treatment services. Also, the misconceptions and myths about cryotherapy are public health concerns. Therefore, the study recommends improved awareness campaigns and service delivery for the enhanced uptake of cryotherapy treatment services.

Objetivo: O objetivo desta revisão de literatura é investigar e analisar criticamente o uso da crioterapia intracanal durante o tratamento endodôntico, visando através dos achados científicos disponíveis elaborar argumentos sólidos que justifiquem optar ou não pela realização de tal protocolo auxiliar durante a intervenção endodôntica. Revisão de literatura: A alternativa terapêutica conhecida como crioterapia é uma técnica existente há anos, a qual tem sido frequentemente aplicada na Medicina para o tratamento da dor e cuidados pós-operatórios. Recentemente, o uso da crioterapia na Endodontia, através de um protocolo de irrigação final do canal radicular, foi examinado em vários ensaios clínicos randomizados. Discussão: Os estudos revisados demonstraram que a utilização de soro fisiológico frio como agente irrigante final ao preparo químico-mecânico, tem potencial para reduzir os níveis de dor de uma possível sintomatologia dolorosa pós-tratamento endodôntico, além de que, trata-se de um método de fácil aplicação e baixo custo. Conclusão: Embora se tenha poucos estudos sobre o tema, as evidências científicas disponíveis e revisadas neste artigo embasam com propriedade o efeito da crioterapia intracanal na redução da dor pós-operatória proveniente de uma intervenção endodôntica. No entanto, novos estudos são necessários para averiguar possíveis efeitos deletérios e para a elaboração de um protocolo previsível e seguro.


Aim: The aim of this literature review is to investigate and critically analyze the use of intracanal cryotherapy during endodontic treatment and, through scientific findings, elaborate well grouded references that justify or not, carrying out such auxiliary protocol during endodontic intervention. Literature Review: The therapeutic alternative known as cryotherapy is a technique that has existed for years, which has been frequently applied in Medicine for the treatment of pain and postoperative care. Recently, the use of cryotherapy in Endodontics, through a final root canal irrigation protocol, has been examined in several randomized clinical trials. Discussion:The reviewed studies showed that the use of cold saline solution as a final irrigating agent for chemical-mechanical preparation has the potential to reduce pain levels of a possible painful symptomatology after endodontic treatment, in addition to being a method of easy appli-cation and low cost. Conclusion: Although there are few studies on the subject, scientific evidences available and reviewed in this article properly support the effect of intracanal cryotherapy in reducing the postoperative pain of an endodontic intervention. However, new studies should be placed to investigate possible harmful effects and to prepare a predictable and safe protocol.

RESUMEN La crioterapia es el conjunto de procedimientos que utilizan el frío en la terapéutica médica. Emplea diversos sistemas y tiene como resultado la disminución de la temperatura de la piel; produce una destrucción local de tejido de forma eficaz y controlada. El objetivo de este trabajo fue realizar una actualización para exponer los aspectos esenciales sobre formas de empleos, indicaciones, complicaciones y contraindicaciones. Existen varios métodos de aplicación de la crioterapia, que incluyen las técnicas de congelación de spray o aerosol y con aplicadores, el método criosonda, y el uso de termoacoplador. Está indicada en varias entidades, entre las que se encuentran la queratosis seborreica y actínica, lentigos solares, carcinoma basocelular y espinocelular in situ. Las complicaciones más observadas son vesicoampollas, hiperpigmentación e hipopigmentación, y las contraindicaciones comunes son intolerancia al frío, tumores con bordes no delimitados o con pigmentación muy oscura, en localizaciones cerca de los márgenes de los ojos, párpados, mucosas, alas nasales y el conducto auditivo. El dominio de los métodos de aplicación e indicaciones es indispensable para elegir la conducta adecuada; de esta forma se evitan complicaciones y efectos colaterales (AU).


ABSTRACT Cryotherapy is the whole of procedures that use cold in medical therapy. It uses various systems and results in a decrease in skin temperature, leading to a local destruction of tissue in an effective and controlled way. The objective of this work is to make an update to expose the essential aspects on the ways of use, indications, complications and contraindications. There are several cryotherapy application methods that include spray or spray freezing techniques and applicators, the cryoprobe method, and the thermocoupler use. It is indicated in several entities, and among the most frequent are seborrheic and actinic keratosis, solar lentigo, basal cell and squamous cell carcinomas in situ. The most observed complications are vesical blisters, hyperpigmentation and hypopigmentation, and the most common complications are: cold intolerance, tumors with non-delimited borders or very dark pigmentation, located near the margins of the eyes, on eyelids, mucous membranes, nasal wings, and on the ear canal. The mastery of the signs and application methods are essential to choose the appropriate behavior against the disease: side effects and complications are avoided that way (AU).

Introdução: O cisto dentígero se origina pela separação do folículo que fica ao redor da coroa de um dente incluso. É o tipo mais comum de cisto odontogênico do desenvolvimento. O seu crescimento é lento, assintomático, e pode atingir grandes dimensões. Objetivo: Relatar um caso clínico cirúrgico de cisto dentígero com transformação ameloblástica, localizado na mandíbula, de paciente, gênero feminino, melanoderma, 14 anos. Caso clínico: Ao exame radiográfico apresentou área radiolúcida unilocular com margem bem definida e esclerótica envolvendo a coroa das unidades 48 e 47. Foi realizada enucleação e curetagem da lesão com exodontia destas unidades sob anestesia local em ambulatório, e aplicada a crioterapia na loja óssea. Encaminhou-se o conteúdo da lesão para exame histopatológico e o diagnóstico de cisto dentígero com transformação ameloblástica foi fechado. Comentários principais: No momento a paciente encontra-se em acompanhamento pós-operatório de 3 anos com neoformação óssea e sem recidivas(AU)


Introducción: El quiste dentígero se origina por la separación del folículo que se queda alrededor de la corona de un diente no erupcionado. Es el tipo más común de quiste odontogénico de desarrollo. Su crecimiento es lento, asintomático y puede alcanzar grandes dimensiones. Objetivo: Reportar un caso quirúrgico de quiste dentígero con transformación ameloblástica. Presentación del caso: Paciente femenina de 14 años, de color de piel negra. La radiografía demostró una radiolucidez unilocular con márgenes bien definidos que envolvían la corona de los dientes 48 y 47. El tratamiento involucró una combinación de enucleación y curetaje de la lesión, exodoncia de los dientes y crioterapia para desvitalizar el hueso circundante. Se realizó el examen histopatológico, luego, se confirmó el diagnóstico de quiste dentígero con transformación ameloblástica. Conclusiones: Al momento de la redacción del reporte la paciente se encontraba en seguimiento posoperatorio de tres años con neoformación ósea y sin recidivas(AU)


Introduction: Dentigerous cysts are caused by the separation of the follicle remaining around the crown of unerupted teeth. They are the most common type of developmental odontogenic cyst. Their growth is slow and asymptomatic, and they may reach large dimensions. Objective: Report a surgical case of dentigerous cyst with ameloblastic transformation. Case presentation: A case is presented of a black female 14-year-old patient. Radiography revealed an area of unilocular radiolucency with well-defined margins enveloping the crowns of teeth 48 and 47. Treatment was a combination of enucleation and curettage of the lesion, exodontia of the teeth and cryotherapy to devitalize the surrounding bone. Eventual histopathological examination confirmed the diagnosis of dentigerous cyst with ameloblastic transformation. Conclusions: At the time when the report was written, the patient had been followed up for three years after surgery, showing bone neoformation and no recurrence of the lesion(AU)

Abstract Introduction: Whole-body cryotherapy (WBC) has shown to be beneficial in the treatment of fibromyalgia (FM). There is cumulative evidence that cytokines play a crucial role in FM. It's unknown whether clinical effects of WBC can be demonstrated at the molecular level and how long the effects last. Methods: We compared effects of serial WBC (6 sessions (- 130 °C in 6 weeks) in FM patients and healthy controls (HC). Primary outcome was the change in pain level (visual analogue scale 0-100 mm) after 6 sessions. Secondary outcomes were a change in disease activity (revised Fibromyalgia Impact Questionnaire) and pain after 3 sessions and 3 months after discontinued therapy and in cytokine levels (interleukin (IL-)1, IL-6, tumor necrosis factor α (TNF-α) and IL-10). The patients' opinions on the satisfaction, effectiveness and significance of WBC were evaluated. Results: Twenty-three FM patients and 30 HC were enrolled. WBC resulted in a significant reduction in pain and disease activity after 3 and 6 sessions. No clinical benefit could be measured 3 months after discontinued treatment. Overall, probands were satisfied with WBC and considered WBC to be important and effective. FM patients had significantly different levels of IL-1, IL-6, TNF-α and IL-10 at each reading point compared to HC. Levels of IL-1, IL-6 and IL-10 were significantly altered over time in FM patients. Compared to HC FM patients showed a significantly different response of IL1, - 6 and - 10 to WBC. Conclusion: Serial WBC is a fast acting and effective treatment for FM. Proven effects of WBC may be explained by changes in cytokines.(AU)

OBJECTIVE@#To compare the clinical therapeutic effect of fire needling stripping after local anesthesia, simple fire needling and liquid nitrogen cryotherapy on verruca vulgaris.@*METHODS@#A total of 900 patients with verruca vulgaris were randomized into a fire needling stripping group (300 cases, 2 cases dropped off), a fire needling group (300 cases, 4 cases dropped off) and a liquid nitrogen cryotherapy group (300 cases, 5 cases dropped off). After local anesthesia of compound lidocaine cream, fire needling therapy was adopted, and the necrotic tissue of verruca was stripped in the fire needling stripping group. Simple fire needling therapy was adopted in the fire needling group, without local anesthesia and stripping. Liquid nitrogen cryotherapy was adopted in the liquid nitrogen cryotherapy group. The treatment was given once a week, and totally 3 weeks were required in the 3 groups. The skin lesion scores of number, area, thickness, color, pruritus, isomorphism and the level of T lymphocyte (CD@*RESULTS@#Compared before treatment, the skin lesion scores were decreased (@*CONCLUSION@#Fire needling stripping after local anesthesia can effectively treat the verruca vulgaris, improve the skin lesion and immunity, its therapeutic effect is superior to simple fire needling and liquid nitrogen cryotherapy.
